When I was in the army, that was at the beginning of the 1950s, they wanted us to leave the faith. I was an atheist, so logically I said to myself that I've got no business being in it. So I filled out the form and gave it back to them. Luckily soldiers are disorganized, I don't know where the form ended up, the community still sends me a deposit slip for a religious contribution before each Jewish New Year, since the amount of the government contribution depends on the number of contributors.
